Insurance & Fees
Insurance coverage depends upon your individual health insurance policy. Please contact your insurance company for details regarding chiropractic coverage: deductibles, copays, and limitations. In the case of Medicare Supplemental policies, you will want to find out if your policy will cover “non-covered” Medicare services. In the event that you do not have insurance coverage or a high deductible plan, we do offer a cash discount. You will not be asked to sign up for a treatment plan or to pay in advance. Please contact us if you have additional questions.
Insurance Plans:
- Dr. Bradley is a Participating Medicare Provider. Medicare covers some chiropractic services, but is limited. Your Supplemental Health Insurance Policy may cover “non-covered” Medicare services. Please contact your insurance company for detailed information.
- We are “in network” for some insurance companies. Please contact our office or your insurance company for “participating provider” information. At your request, we will submit a claim to any health insurance plan regardless of participation status.
- A referral from your MD is not required for Chiropractic care.
If treatment is for a motor vehicle accident, make sure to bring:
- The name, address and phone number of your insurance company.
- Your claim number.
- The name of your claim adjustor and phone number.
- The name, phone number and address of your attorney (if applicable).
- We do not accept liens.
If treatment is for a work related injury, make sure to bring:
- The name of your supervisor or human resource manager and phone number.
- Your claim number.
- The name, address and phone number of your employer’s Worker’s Compensation Insurance Company.
- The name of your claim adjuster and phone number.
- You have two choices of providers licensed in the state of Wisconsin. These are your choices, not your employer’s.
